In completing this form, please be aware of the following:-

1. It is a legal requirement for you to make an application for admission of a pupil to a school.

2. The application must be made to the Admissions Authority.

3. Carmarthenshire County Council is the Admissions Authority. A community school cannot admit or guarantee a place for a pupil at a school. Only the Admissions Authority can do so.

4. If you are offered a place at a school then you must return the acceptance form by the deadline specified in the offer letter or the place will be withdrawn.

The closing dates for the next normal round of admissions to schools for the period September 2012 to April 2013 for Nursery and Primary schools is January 31st 2012 and for the return of forms relating to the primary to secondary school transfer the deadline date is the 3rd February,2012.

This form can also be used for applications for admission to schools at other times of the year but they will be regarded as late applications and will not have the same priority as those received before the deadline dates.

Fraudulent Application.

Where parents knowingly give false information in order to obtain the advantage of a particular school to which they would not normally be entitled, they may render themselves guilty of a criminal offence. Places may be withdrawn if such declarations are subsequently found to be incorrect.

Data Protection Act 1998

The information which you provide on this form will be used for the administration of school admission / transfers, school health service, statistical monitoring purposes and will be used by the Council to verify residency. Carmarthenshire County Council is registered under the Data Protection Act 1998.

Date of Return / Late Admissions

All applications for admission during an academic year must be received by 31st January the previous year e.g. all applications for admission from September 2012 to July 2013 must be received by 31st January, 2012. Applications made after the published deadline may not be included and may therefore mean that the preferred school is not available. Applications to schools outside the normal admission cycle will be considered in the same way as those applying for entry at the usual time.
